Home Nursing Care in Malaysia - Services, Costs & What to Expect

Private care homes in Malaysia publish monthly fees from about RM1,500 to RM5,900. In the three full price lists in this guide, checked 14 October 2026, a shared room costs RM2,000 to RM4,500 or more and a private room RM2,150 to RM5,900. Premium assisted living starts from RM10,800 a month. Government homes take residents through a JKM application. What your parent pays depends on the room, the care they need and the extras charged on top.
If you are pricing care for a parent, you have probably noticed that every home quotes differently and few say what the fee leaves out. The calculator below turns published prices into a monthly and one-year estimate for your parent's room and care needs.

Estimate based on three homes' published prices, checked 14 October 2026. Genesis Life Care publishes starting fees, so its actual fee can be higher. One year is 12 months at today's fees, before deposits, fee rises and one-off charges.
Look at the gap line in the result. For the same room type and care need, the gap between the cheapest and most expensive home is RM1,400 to RM3,450 a month. The home you choose, and the exact room within it, can move the bill as much as your parent's care needs do.
Nursing home prices in Malaysia by type of home
The examples below are prices providers publish on their own websites, grouped by type of home. Prices change, so confirm with each home. Care Concierge publishes this guide and runs assisted living residences; its own prices are on its residences page.
Monthly fees by type of home
Each bar runs from the lowest to the highest fee in the three full price lists. Genesis Life Care publishes starting fees, so its top fees can be higher.

Assisted living residences
Monthly fee
Varies by residence, room and care level. Ask for a written quote.

Monthly fee
From RM10,800.
What you usually get
A private suite with housekeeping, laundry and care support, beside Sunway Medical Centre.
Example
Sunway Sanctuary in Subang Jaya.
JKM describes Rumah Seri Kenangan as a last resort for seniors who are neglected and need protection. Applications go through a district welfare office and depend on vacancies. A parent with working children does not meet JKM's no-family condition, so plan for a private home.
Sunway Sanctuary's website gives two starting prices: RM10,800+ a month for assisted living in its FAQ and RM13,300+ on its living page. Ask which applies to the suite you are shown.
What pushes the price up or down
Room type
Sharing a room lowers the fee. At Seavoy Nursing Home in Kuala Lumpur, a resident who walks and eats with little supervision pays RM2,000 a month in a ward of six to eight beds, RM3,400 in a two-bed room and RM5,200 in a large single room. Genesis Life Care charges from RM2,500 for a shared room and from RM5,000 for a private room at any care level.

Shared room
RM2,000 to RM4,500 or more

Private room
RM2,150 to RM5,900
Level of care
The more hands-on help your parent needs, the higher the fee. Seavoy prices five care categories, from residents who walk and eat with little supervision to residents who are bedridden and need intensive nursing and physiotherapy. In a small single room, those categories run from RM3,900 to RM4,900 a month. Genesis Life Care's starting fees step up from RM2,500 for essential care to RM3,000 for assisted living, RM3,800 for nursing or high-dependency care and RM4,500 for complex and palliative care.

The home and its area
The Johor Bahru price list in this guide is lower than the Klang Valley ones. Spring Valley Homecare in Kempas Baru charges RM2,000 to RM2,550 a month across all its rooms and care levels, against RM2,000 to RM5,900 at Seavoy in Kuala Lumpur.
Who is on duty
Homes describe their staffing differently: caregivers only, caregivers supported by nurses, or a nurse on site. Ask who is on duty at night, and whether a nurse is in the building or on call. This affects both the fee and whether the home suits your parent's condition.

What the monthly fee covers, and what costs extra
At a residence, the monthly fee normally covers the room, meals, help with daily activities such as bathing and dressing, housekeeping and laundry. Care Concierge's own brochures list:
- The Mansion: professional caregivers for daily activities and companionship, a senior nurse on site, an occupational therapist, two meals a day cooked by a professional cook plus breakfast, tea and snacks, housekeeping and laundry, and the Care Concierge app for health and vital records. At The Mansion Ampang, professional caregivers supervise residents round the clock.
- Tanjung Point Residences: professional caregivers round the clock, supervised by nurses and other specialists, social and cognitive activities, all meals, Astro and Wi-Fi, and weekly housekeeping and laundry.
- Angsana Home: trained caregivers round the clock, registered nurse support, daily programmes led by a physiotherapist and caregivers, all meals, Wi-Fi, and daily housekeeping and laundry.

What often costs extra
Genesis Life Care states that consumables such as diapers and feeding formula, specialist medication, doctor or specialist visits and physiotherapy are charged separately. Spring Valley Homecare in Johor Bahru publishes its full list of extras, including RM60 a physiotherapy session, RM90 for transport within 10 km, RM300 a month for an oxygen concentrator, and diapers by the pack. Seavoy in Kuala Lumpur charges RM200 for a doctor's emergency call. In-house physiotherapy costs RM50 an hour at City Heart Care in Johor Bahru and RM90 a session at Golden Years Senior Residence in Selangor.
Physiotherapy twice a week at RM50 to RM90 a session adds RM400 to RM720 a month, and four diapers a day at Spring Valley's pack prices add about RM180 to RM310. Add the extras your parent will need before you compare monthly fees.
Deposits and one-off costs
Spring Valley Homecare asks for two months' fees, refundable if you give one month's notice before your parent leaves. Golden Years Senior Residence takes a RM3,500 booking deposit, refundable on discharge. Seavoy charges RM460 for an admission medical check: a doctor's first consultation, a blood test and an ECG. Count these in your first month's budget.
A first year, worked through
Take a double room at Spring Valley Homecare in Johor Bahru, using only its published prices, for a parent who needs help every day: RM2,200 a month, or RM26,400 for the year. Add physiotherapy twice a week at the home's RM60 a session (RM5,760 a year) and four diapers a day at its pack prices (about RM2,160 to RM3,720 a year). The first year comes to about RM34,320 to RM35,880, which is 30% to 36% more than the room and care fee alone. The deposit of two months' fees, RM4,400, is paid up front and refunded if you give one month's notice.

Nursing home or home care: which costs less?
A residence fee usually bundles accommodation, meals and daily support. A quote for care at home covers agreed care hours; the household still pays for food, utilities, equipment and any hours not covered by family or the care team.

Compare two written quotes for the same needs and schedule. A few hours of help at home can cost less than moving into a residence. A parent who needs support throughout the day and night needs a different comparison: check active care hours, rest periods and overnight cover before treating a live-in arrangement as continuous care.
Our home care cost guide, live-in caregiver guide and home care versus residence comparison explain the questions to ask. Care Concierge gives a personalised home care quote after assessing the care your parent needs.
How families pay for nursing home fees

Income tax relief for parents' care
For year of assessment 2025, the latest year on LHDN's relief list, eligible taxpayers may deduct up to RM8,000 for medical treatment, dental treatment, special needs and carer expenses for parents and grandparents. Complete medical examinations, including vaccination, count within a RM1,000 limit. LHDN's public ruling includes care services provided by a care home, and care at home or in any care centre.
- A doctor registered with the Malaysian Medical Council must certify that your parent's condition needs treatment or a carer.
- Your parent must be resident in Malaysia, and the care must be provided in Malaysia.
- The deduction is on expenses you paid yourself, so keep receipts in the name of the child who pays.
Tax relief reduces taxable income; it is not an RM8,000 cash payment, and not every part of a residence bill automatically qualifies. Before you file, confirm with LHDN or a tax adviser which expenses and documents qualify and, if siblings share the fee, how each of you should claim.
EPF Akaun Fleksibel (Account 3)
If you are below 55, you can withdraw from your EPF Akaun Fleksibel at any time, with a minimum of RM50 per withdrawal, through the KWSP i-Akaun app, the i-Akaun portal or an EPF office. Withdrawal reduces your retirement savings. Check the available balance and your longer-term budget before using it for care fees.
Insurance
Before you count on insurance, ask your parent's insurer in writing whether any part of a residence stay is covered, and under which benefit.
Questions to ask before you sign
Ask each home for these answers in writing:
- How much is the deposit, and is it refundable when your parent leaves?
- Is there an admission or registration fee, and what does it cover?
- What does the monthly fee include: meals, laundry, activities, the care team?
- What costs extra: diapers, medication, doctor visits, physiotherapy, hospital escorts, special diets?
- Who is on duty overnight, and is a nurse on site or on call?
- How does the fee change if your parent needs more care later?
- How much notice must you give to leave, and what happens to fees during a hospital stay?
- Can your parent try a short stay before committing?
- Will receipts itemise care services clearly enough for a tax relief claim?
Before you call a home: a five-step plan
Step 1
Score your parent's daily needs with the free Barthel Index assessment, so every home quotes for the same care level.
Step 2
Use the calculator above to set a monthly and one-year budget, with the extras your parent is likely to need.
Step 3
Ask three homes for an itemised quote at that care level and room, with the deposit and notice terms.
Step 4
Compare the one-year totals, extras included, rather than the headline monthly fee.
Step 5
Visit your shortlist twice, once at a meal time. Our guide on how to choose a care home in Malaysia lists what to check.
Get a quote and visit a residence
Getting help isn't giving up.
Care Concierge runs assisted living residences in the Klang Valley and Penang: The Mansion in Petaling Jaya and Ampang, Care Luxe in Kuala Lumpur, and Tanjung Point Residences and Angsana Home in Penang. A Care Advisor will ask about your parent's needs, explain what each residence would cost for them, and arrange a visit, with no pressure and no obligation. Current prices are on our residences page.
Ask a Care Advisor for a quote, or compare with care at home if staying home matters most. Care Concierge also provides care at home in Johor and Perak, where it has no residence.
