Clock Drawing Test
An assessment for cognitive function, particularly executive and visuospatial skills.
Assessment Steps
Follow the instructions and enter the score for each criterion below.
Step 1: Preparation
Provide the Patient with a blank piece of Paper and a Pen/Pencil.
Step 2: Instruction
Instruct the Patient: "Please draw a clock, put in all the numbers, and set the time to 10 past 11." Repeat only once if necessary.
Step 3: Patient Draws Clock
The patient can use the drawing pad below to draw the clock. The drawing is for reference and will not be saved.
Step 4: Observe and Score
Clock Face
2 Points: Circle is closed and round.
Numbers
1 Point: All numbers (1-12) are present.
1 Point: Numbers are in the correct order.
1 Point: Numbers are placed correctly on the clock face (roughly in correct quadrants).
Hands/Time Setting
2 Points: Correct time set to 11:10 with the hour hand shorter than the minute hand.
1 Point: Minor errors in time or hand length.
